How to Send a Survey in Power Automate Using FlowsTo send a survey in Forms Pro from your Microsoft Dynamics 365 environment, there are a few simple steps you need to follow.

While we covered a few of the basics in previous blogs, this blog will break down the process into more detail for you and your team to help you utilize more intricate functionality like flows and triggered bursts.

Once you’ve gone through the steps of creating your survey in Forms Pro, you’ll reach the point of sending.

For a basic survey, you may use a QR code for mobile users, a survey embed code to put on a website or other appropriate landing page, a link for social media and networking, or click on the email icon to send using your Outlook client.

You may also use more advanced functionality to send your survey out from Forms Pro.


First, you must build your Flow through Power Automate.

  1. For those of you familiar with Power Automate, there are many ways to start a Flow (Trigger/Automated, Instant, Scheduled, etc.).  For this example, we are going to create a Flow that can be called from inside D365.
  2.  Login to D365 Sales/Customer Service/Field Service and go to the Entities views (This example is using Contacts).  From the Task menu, select Flow and Create a flow

  3. This will redirect you to Power Automate and start you with a When a record is selected Step and the ability to select the starting Entity Name (Since we started from Contacts, it should say Contacts)

  4. Now you add a Step to get fields from the Contact records through a Get record Step and then you are able to send a survey through the Send a survey Step.  From the Send a survey Step, you are able to populate all your variables that you set within Forms Pro to the values/fields that are available with the D365 entity (Contact for this example)

Now, through D365, you are able to call the Power Automate Flow. To utilize Flows for sending a survey, follow these steps:

  1. Inside of Microsoft Dynamics 365, open your Contacts.
  2. Navigate to your Active Contacts. For this example, we’ve created our own “My Active Contacts – Survey” list to put it into perspective.

  3. Select your contacts, and then click on the ellipses in the upper right-hand corner of your screen. A dropdown menu will appear. Click on “Flow.”

  4. “Flow” is for Power Automate, or what used to be called MS Flow. You will see a new menu appear. Click “Send Survey” under “Run.”

  5. A pop-up will appear asking if you want to run the flow. Click “Run flow.”

  6. A confirmation screen will appear to let you know the flow has successfully been run. Click “Done.” Behind the scenes, Microsoft Dynamics 365 has run a power automate flow to send out the survey to your selected contacts or contact list.

Setting up a triggered burst is fairly simple to set up in Power Automate if you have familiarity with it.

You can have your survey triggered based on different events that happen inside of your Microsoft Dynamics 365 environment.

So, for example, if you have a case that comes in and it becomes resolved, and based on this you want to send a survey requesting feedback on the case resolution process and satisfaction, you can do that.

You can also set up triggered bursts based on frequency, so if you want to ensure an individual doesn’t get more than one survey from you in a 30-day period.

Microsoft* provides five steps for using Power Automate to send a survey:

  1. Open the survey that you want to send, and then click “Send” from the toolbar at the top of your page
  2. Click “Power Automate,” and then compose your email
  3. Click “Configure Flow”
  4. In order to configure a built-in-flow, click on a template. Learn more about the steps to do this here.
  5. To configure a flow from scratch, click on “Create from blank.” Learn more about the steps to do this here.
  6. Once you’ve finished the steps in 4 or 5, click “Create Flow.”
